At Catholic Metro Sports, the safety and well-being of our community are our top priorities. We are committed to providing a positive and safe environment for all participants, whether they are players, coaches, volunteers, or spectators.
Purpose of the Incident Report Form:
This form is designed to report any issues or concerns related to games, practices, or league activities. It is an important tool for us to address and resolve incidents promptly and effectively.
When to Use This Form:
- If you have witnessed or experienced an incident that raises concern.
- If there are issues that need to be addressed regarding the conduct of participants, coaches, or spectators.
- To report any other concerns that may impact the safety or integrity of our sports programs.
Immediate Concerns:
If you encounter any immediate concerns regarding safety or well-being during an event, please address these with the individuals who are in control of the event immediately. In situations where there is a threat to personal safety or well-being, do not hesitate to contact local authorities.
If you need to report actual or suspected child abuse, please call in the following order:
1) Department of Family & Children Services (DFCS) at 1-855-422-4453
2) The Archdiocese of Atlanta 24-Hour Reporting Line at 1-888-437-0764
If a child is in immediate danger, call 911.
Normal Incident/Concern Reporting Chain:
Incidents should typically be reported to and in the following order:
- The Coach
- The Sport Director
- The School/Parish League Representative
- Catholic Metro Sports A list of contacts is located here on our website. If for any reason the Principal or the Pastor needs to be contacted this will be done by the School/Parish Representative or by Catholic Metro Sports.

Please complete the Incident Report Form and submit it via email to info@catholicmetrosports.com. Our team will review the report and follow up accordingly to ensure that appropriate actions are taken.
